Community Health Needs Assessment

Nonprofit hospitals are required under IRS §501(r)(3) to complete a Community Health Needs Assessment every three years. A CHNA identifies and documents health needs and gaps in care within a defined service area, using demographic, socioeconomic, and health status data along with community input. VMG Health conducts data review and analysis, engages community stakeholders for resident and stakeholder feedback, and produces the assessment report.

Implementation Strategy Report

Implementation Strategy is where assessment findings become commitments.  Our experts assist organizations in developing a board-adopted plan that specifies which health needs will be addressed, how it will address them, resources allocated, and anticipated impacts. VMG Health helps leadership move from a list of prioritized needs to a strategy that’s realistic, resourced, and ready for adoption.
