The Rural Health Transformation Program (RHTP) is a $50B federal investment, directed by the Centers for Medicare & Medicaid Services (CMS), to improve healthcare access, sustainability, and quality in rural areas of the U.S. From 2026 to 2030, it funds state-level initiatives for care innovation, workforce development, and technology implementation. 

In December 2025, CMS approved all 50 states for funding. First-year awards averaged roughly $200 million per state, ranging from $147 million to $281 million. The $10 billion available annually is split in two halves: 50% distributed equally among approved states, and 50% allocated based on rural population share, the number and needs of rural health facilities, and other factors CMS identifies.

RHTP money does not flow directly from CMS to providers. Each state receives its allotment and then decides how to distribute it—largely through subawards, subgrants, and subcontracts to hospitals, health centers, and community organizations. Every state sets its own priorities, eligibility rules, and timelines.

Funding runs in annual budget periods through 2030. Budget Period 1 began December 31, 2025 and ends September 30, 2026; subsequent periods begin October 31 each year. States are working against a deadline to obligate first-year funds, which is why solicitations have come quickly this spring and summer, and why a new wave is expected as the next budget period opens. A state with nothing posted today has money and a plan; its round simply hasn’t been released yet. Every state below includes its allocation and, where available, exactly where to watch.
